Minimally Invasive Surgical Techniques for Low Rectal Cancer
Since 2000, led by Professor Zhou Zongguang, the gastrointestinal surgery team at West China Hospital has pioneered minimally invasive surgical treatments for low rectal cancer—both domestically and internationally—while simultaneously advancing research on micrometastasis in tumor regions. This groundbreaking work has broken through the longstanding limitations of laparoscopic techniques in treating low rectal cancer, addressing critical TME-related challenges that had long hindered treatment efficacy. As a result, the team has precisely defined the regional resection boundaries for minimally invasive TME procedures, significantly enhancing surgical outcomes while dramatically reducing surgical trauma. Thanks to these innovations, low rectal cancer patients who previously faced a 75% risk of requiring permanent colostomy or ileostomy can now enjoy a much higher chance—up to 85%—of achieving functional anus-preserving surgery. Consequently, their postoperative quality of life has markedly improved. Additionally, the amount of blood transfused during surgery has been reduced substantially: from an average of 300–900 ml per procedure to nearly 90% of patients requiring no transfusion at all—a level that places West China Hospital’s approach firmly at the international forefront.
Building on this foundation, the gastrointestinal surgery team has successfully pioneered several innovative techniques, including laparoscopic lateral lymph node dissection for low rectal cancer. By employing precise preoperative assessments, they’ve enabled personalized lateral lymph node clearance, significantly reducing local recurrence rates while preserving anal function. Another breakthrough is the laparoscopic extralevator abdominoperineal excision (ELAPE) performed without patient repositioning, which, compared to the traditional MILES procedure, lowers the risk of specimen perforation and circumferential margin positivity, while also streamlining the surgical process and shortening operative time. Additionally, the team has introduced open small-incision functional low rectal cancer resection, modified posterior approach for locally recurrent cancer resections, and regionally advanced cancer surgeries featuring functional expanded resections—all aimed at advancing rectal cancer treatment toward greater functionality, minimal invasiveness, and bloodless procedures. As a result, both the volume and quality of their surgical outcomes now rank among the highest in China and globally, delivering outstanding results that combine radical tumor removal with excellent functional preservation, including successful sphincter-sparing techniques. The Gastrointestinal Surgery Center has become the only institution in China accredited simultaneously by both the American College of Surgeons (ACS) and the Royal College of Surgeons of Edinburgh (RCSEd) as a Clinical Skills and Surgical Training Center. To date, the team has completed over 10,000 minimally invasive rectal cancer surgeries, averaging more than 1,000 cases annually. Their sphincter-preserving rate stands at 85%, with five-year overall survival rates reaching 90% for Stage I patients and 80% for Stages II–III.
